What the research says about vinegar and blood sugar

Vinegar has been studied by nutrition researchers for decades, and one question comes up more than any other: what does a little acetic acid do to the rise in blood sugar that follows a meal? Below is a plain-language look at what the published literature actually reports, with the original papers listed at the end so you can read them yourself.
This is a summary of research on vinegar, not a claim about any product, and it is general information rather than medical advice. If you manage a blood sugar condition or take medication, your clinician is the right person to talk to.
The basic finding
Across a number of small human trials, taking vinegar with or just before a carbohydrate-rich meal has been associated with a smaller rise in blood glucose in the hour or two afterward, compared with the same meal without vinegar. The effect sizes vary between studies, and the trials are small, but the direction of the finding has been fairly consistent since the early 2000s.
One frequently cited example is a 2004 trial by Johnston and colleagues, which reported reduced post-meal glucose and insulin responses in participants who took vinegar before a high-carbohydrate meal. A 2010 study by Mitrou and colleagues looked at the same question in people with type 2 diabetes and reported a similar pattern.
Why researchers think it happens
There is no single agreed mechanism, but the literature offers a few candidates, and they are not mutually exclusive:
- Slower stomach emptying. Acetic acid appears to delay how quickly a meal leaves the stomach, which spreads carbohydrate absorption over a longer window.
- Enzyme interference. Acetic acid may partially inhibit enzymes that break starch down into simple sugars, so less glucose arrives at once.
- Muscle glucose uptake. Some studies report improved glucose uptake in skeletal muscle after vinegar, though this work is earlier-stage.
What the research does not show
It is worth being precise about the limits. Most of these trials involve a few dozen participants at most, measure a single meal, and use a defined dose of vinegar, usually one to two tablespoons diluted in water. They do not show that vinegar treats or prevents any condition, they do not measure long-term outcomes, and they are not a substitute for the things with much larger evidence bases: what you eat overall, how you move, and how you sleep.
Reviews of the field tend to reach the same conclusion, that the short-term glucose findings are interesting and reasonably reproducible, and that larger, longer trials are still needed.
